Irinotecan is a prodrug, which is activated to SN-38 by cleavage of the side chain by the enzyme carboxylesterase (CE) in plasma, intestinal mucosa, liver and tumor cells

Uridine-diphosphate-glucuronosyltransferases catalyze the inactivation of SN-38 to the glucuronide form (SN-38G). The inactive form (SN-38G) is excreted with the bile into the intestinal lumen, where its conversion back to the active form (SN-38) is catalyzed by enteric bacterial beta-glucuronidase
